Provider Instructions

Thank you for partnering with REPS to support mental health in our community. Here’s what you need to know to participate in our counseling program:

1. Review the Provider Contract

Please review and sign the Provider Contract before seeing clients.

2. Schedule Sessions

Once REPS approves a client, they may contact you directly. Confirm they’re a REPS Referral before scheduling.

3. Provide Counseling

Offer up to 5 sessions.

4. Submit the Reimbursement Form

Complete a Payment Reimbursement Form per client, as often as you'd like (e.g., after each session, monthly or after all 5 sessions).

  • Upload an invoice including the client and number of sessions

5. Submit Final Session Evaluation

After the 5th session, fill out a Final Session Evaluation Form to help us track outcomes and determine follow-up needs.
