The Health Partnership- Routt, Moffat, and Rio Blanco County
The Health Partnership connects individuals facing barriers such as poverty, isolation and health issues, with the resources they need to achieve health and well-being. We identify systems level gaps and facilitate collaborative and equitable conversations with community partners, healthcare organizations and individuals to identify and test equitable community driven solutions to our most thorny challenges.
Integrated Community (CIIC) was formalized in 2004, but its roots began earlier, when immigrants in Northwest Colorado started gathering in a local bookstore for English conversation groups. It was one of the only places Spanish-speaking residents could connect, learn, and help each other navigate unfamiliar systems.
In response to these emerging needs, over 75 community members, including immigrant representatives, government officials, healthcare providers, nonprofit leaders, and neighbors, came together to listen, share, and prioritize solutions.
One need rose above the rest: without language support and system navigation, families could not access education, medical care, or legal help.
Integrated Community- Routt & Moffat County
Founded in 1983, Advocates began providing services for victims of domestic violence. Since then, Advocates launched a 24/7 crisis hotline, established a confidential safehouse for victims fleeing abusive situations, expanded services to victims of sexual assault, and developed a robust social change program to prevent violence before they happen.
The devastating impacts of violence on individuals and the community cannot be overstated. Beyond providing support for victims and survivors as they navigate healing, medical, and legal processes, Advocates also collaborates with schools, businesses, and organizations throughout the community. With greater awareness and developing necessary skills together, we can improve equity and make everyone safer.
